How is the doctor-patient relationship different here, compared to other hospitals or clinics?

We believe that our staff members are well trained in providing patient-focused care. We take the time to listen to our patients’ concerns and address any questions they may have. We value all of our patients and appreciate the long-lasting relationships we create with them. The patients under our care trust us and the majority of patients have been part of our practice for more than 10 years, some for more than 20 years.

What are your office hours of operation?

We are open Monday through Friday from 9:00am to 5:00pm.

Will I be able to get copies of my test results?

Yes, upon request we are able to provide you with copies of your test results.

Do you allow urgent or same day appointments?

Yes, we do set aside time for urgent visits. However, if you are having an emergency, please dial 9-1-1 or go directly to the nearest emergency room.

How do you notify your patients of lab or test results?

On the New Patient Form that you fill out on your first visit, there is an opportunity to answer how you would like to be notified regarding any test or lab results. We are usually able to notify patients within a week of receiving results.

How is my personal and medical information protected?

Our practice takes your privacy very seriously. Our office is compliant with federal, state and HIPAA regulations. A copy of our office policies is available upon request. If you have any questions, please feel free to discuss these issues with our staff.

What is your cancellation policy?

We require a 24 hour notice to cancel all appointments. Please be sure to contact us if you will not be able to make your scheduled appointment to avoid having to pay a cancellation fee. If something urgent comes up and you are unable to provide 24 hours notice, please contact us as soon as possible and we will try accomodate your needs.

How can I get a hold of my medical doctor?

We take pride in our doctor-patient relationships and schedule plenty of time for you to get all of your questions answered. If you need to get in touch with Dr. Thukral in between appointments, please contact our office at 408-258-7400 to leave a message or schedule an appointment.

How will I pay for my care?

If you have insurance, please bring your card with you to every visit. You may have a co-pay associated with your insurance plan, we will collect this at the time of your visit. If no co-pay is required, we will bill your insurance on your behalf. If you do not have health insurance coverage, payment will be due at the time of service. We strive to have all aspects of our practice, including our professional fees, reflect our ethical and rational approach to your care.

Do I need a referral to make an appointment?

No, we are currently accepting new patients and it is not necessary to have a referral in order to make an appointment with our office. However, for your visit to be covered through your insurance plan please contact your insurance provider to understand the terms of your policy. In case of HMO insurance plans, if you are currently assigned to a different primary care provider, you must change the provider to Dr. Thukral in order for the visit to be covered. We can assist you in the process of selecting Dr. Thukral as your Primary Care Physician.

Is there anything I can do to prepare for my first appointment?

Please make sure that you arrive to our clinic at least 15 minutes before your scheduled appointment time. This allows us the opportunity to complete the registration and insurance verification process, which will help us get you in to see the doctor with minimal wait time. To save time, you can also print and complete the New Patient Form ahead of time and bring them with you to your appointment. If you are under specialist care, please bring your doctors contact information with you. If you are currently taking any prescription medications, please bring all your medications (in their original containers) with you to every visit.

Do I need to have my blood pressure checked regularly?

It is recommended that your blood pressure be checked every two years after the age of 21. We also recommend annual check-ups with your primary physician after the age of 50 and frequent check-ups if you have additional risk factors. The most common risk factors for high blood pressure include family history, being overweight and having a high sodium or poor diet.
